Semaglutide is a game-changer for people who have high blood pressure, diabetes, or high cholesterol due to being overweight. What Is Semaglutide?

Semaglutide is an agonist, which means it mimics the action of a naturally-occurring hormone. This hormone, known as GLP-1, plays a role in regulating blood sugar, appetite, and digestion. More specifically, the GLP-1 hormone reduces appetite and increases insulin to reduce blood sugar. As an agonist, semaglutide triggers the same actions to lower blood sugar and decrease appetite.

How Semaglutide Works

Semaglutide works by affecting specific hormones produced by the pancreas. These hormones send signals about feeling of hunger and satisfaction to the brain. More specifically, the hormones latch onto receptors found in the brain, pancreas, and other areas of the body – binding to these receptors causes changes in the body that reduce appetite and more.

GLP-1 Receptor Agonists

Semaglutide belongs to the class of drugs known as glucagon-like peptide-1 (GLP-1) receptor agonists. The use of these drugs has grown substantially worldwide.

This class includes several different injectable medications administered through a needle, and as a tablet that the patient takes by mouth. There are drawbacks to these injectables and tablets, though. Many patients would prefer to avoid injections, for example, and the digestive tract does not absorb the medication well from the tablets.

Because of these disadvantages and for other reasons, prescribers and patients may prefer a patent-pending compounded formulation of semaglutide.

Peaks and Troughs

Normally administered through an injection or pill, semaglutide levels can have peaks and troughs, or very high levels immediately after administration and very low levels just before the next dosage. It can also take several weeks for the medicine to reach a steady state, in which the peaks and troughs normalize to create a therapeutic level that does not fluctuate.

The peaks and troughs also make it difficult to titrate, or increase/decrease, the dosages. When patients start taking semaglutide, they usually start on a lower dosage to avoid stomach upset. The doctor may increase the dosage after 4 weeks and adjust the dosage every 4 weeks.

Taking small dosages more often, such as once a day instead of once a week, can reduce the difference between peak and trough drug levels compared to a once-a-week dosage. Decreasing the highs and lows of peak and trough drug levels can help smooth out dosage adjustments. It also helps balance steady-state concentration, which is the amount of drug leaving the body with the amount of semaglutide entering the body. What Is Sublingual Semaglutide And How Is It Different?

Sublingual (SL) semaglutide is a compounded suspension of semaglutide taken sublingually, or under the tongue. We designed our patent-pending formulation with a special base capable of carrying large molecules, like those that make up semaglutide, through the oral tissues of the mouth. This compounding base is self-emulsifying, which means it suspends beads of the active ingredient in liquid. The base also has special properties that help hold it onto mucosal tissue long enough to be absorbed completely and then pass easily through oral tissues into the body.

What Is The Dosage And How Is It Supplied?

An important consideration when titrating the dosage is that semaglutide has a half-life of approximately 7 days, which means half of the medicine has left the body by the time the patient receives another dosage. This half-life affects how quickly the patient will reach steady state – with semaglutide, it will usually take 4-5 weeks to reach steady state drug concentrations. Also, since the dosages are daily instead of weekly, patients may experience a smoother titration and reach steady state concentration faster.

When adjusting the dosage, it's important to note that semaglutide stays in the body for about 7 days and usually takes 4-5 weeks to reach steady drug levels. With daily dosages instead of weekly ones, patients may find it easier to adjust gradually and reach steady levels smoothly.

The starting dosage for semaglutide is usually 0.25 mg injected beneath the skin once a week for four weeks. This lower dosage helps build levels of semaglutide and helps patients reach a steady state. Doctors then typically increase the dosage every 4 weeks, with a typical maximum dosage of 2.4 mg once a week. Following each dosage change, it can take as long as 4 to 5 weeks to reach steady state.

In sublingual form, Semaglutide comes in an amber bottle along with a 1 ml dropper for easy under-the-tongue administration. This drug needs to be stored at room temperature, and shaken thoroughly before each use. The patient should apply the dosage under the tongue and hold it for at least 5 minutes before swallowing, and avoid eating or drinking anything for at least 30 minutes.

Researchers are currently conducting a number of studies that are providing a wealth of information about semaglutide in general and the patent-pending sublingual formulation as well. Please check back often, as we will share information about Semaglutide and GLP-1 receptor agonist medications in general.

The most common side effects include gastrointestinal related symptoms such as abdominal pain, constipation, diarrhea, nausea, and vomiting. These side effects may be more common when the patient first starts semaglutide and when titrating to stronger dosages. Extreme sensitivity reactions, such as rash, trouble breathing, swelling of the mouth, face, or throat can occur and will require immediate medical attention. Low blood sugar can occur, especially if taking medication for diabetes. Discontinuing semaglutide may be necessary if these reactions occur.

Is Sublingual Semaglutide Approved by the FDA?

The Food and Drug Administration does not approve compounded drugs, which are custom-made medications made from FDA-approved medications. Because the medications used in the compounded formulation of sublingual semaglutide are already FDA-approved, sublingual semaglutide formulations are not FDA-approved.
